As healthcare distances itself from the traditional fee-for-service care model and moves toward maximizing value for patients, individual health clinics—especially primary care clinics—are faced with challenges such as: staying up-to-date with evolving industry knowledge, lack of access to relevant training and development opportunities, implementing innovative changes inside complex health systems, and lack of information about transformation efforts happening around them.

At Mount Sinai’s Arnhold Institute for Global Health, our Health Systems Design team is working to leverage and support existing primary care communications through a learning network that addresses the needs of primary care practice dyad leadership (Practice Managers and Medical Directors) and provide access to the knowledge and training they need to be successful in their practice transformation efforts and day-to-day operations.

We have engaged our end users—the dyad leadership in the system’s 52 primary care clinics—in participatory research, contextual interviews, and quantitative surveys, and we have synthesized that research into various opportunities for designing a thriving, seamless online / offline-experiential network of primary care professionals who are advancing their clinics through value-based care.

In our talk, we would like to share our process of bringing human-centered design to the primary care landscape and scaling it up to all of Mount Sinai’s 52 primary care clinics, the challenges and opportunities in designing with a large health system, and our hypothesis that re-equipping the existing primary care workforce for value-based care transformation will help the health system meet its desired health and financial outcomes.
