Helping People Afford Their Meds No Matter What It Takes (As A Payer!)

As a payer, time and time again we face patients who are not taking critical medications because they are too expensive. We know that taking those medications is what keeps them out of the hospital and in their homes. By leveraging our clinical pharmacy and care management organizations, we implemented a cost assistance program to help patients get access to their medications by whatever means available (grants, funds, patient assistance programs, etc.) by serving those in need with a dedicated Community Resource Specialists. We proved patients desire this type of service and revealed the opportunity size present within our post discharge population. We used Innovation processes to frame, discover, and analyze the tangible needs of our patients, and executed a concept that helps them access healthcare and experience improved outcomes. In our first test and learn we saw an average out of pocket savings of over $3,000 for patients who participated. We have learned about how social services can impact health as much as clinical care and are excited to continue our journey toward becoming a healthcare company.
